The Office of the Seniors Advocate’s Residential Care Survey asked residents in all of B.C.’s publicly-funded residential care homes questions on a variety of functions and experiences that happen on a day-to-day basis in care homes, from questions about food quality and staff responsiveness to observations of physician care and social connectedness. The survey was conducted between June 2016 and May 2017. Residents’ most frequent visitors (MFVs), usually a family member, were also mailed surveys. More information about the survey may be found in the Office of the Seniors Advocate’s September 2017 provincial results report.

The facility-level results below are presented alongside B.C.’s provincial-level results for each question. Most questions were asked of both the resident and their MFV; for these questions, the residents’ responses are presented first, and the MFVs’ responses are given in brackets. Some questions were asked only of the residents; in these cases, no MFV responses will be displayed. Some questions, in turn, were asked only of MFVs; these questions are identified as such under the banner of “Most Frequent Visitor-only questions” and do not have resident responses displayed.

The first set of questions asked residents and their MFVs to rate overall aspects of their care experience, including quality of care, willingness to recommend the care home to others, and the cleanliness of the care home.

Personal Control

This group of questions is designed to gauge the sense of personal control or autonomy residents feel they have over their day-to-day life in the care home. Being able to do what we want, when we want, is important to everyone regardless of age or where we live. However, we may take for granted the many individual day-to-day decisions we make for ourselves that, in a care home, are dictated by policy, regulation and resource allocation decisions made by other people.

Social Life

Social engagement is increasingly being recognized as a significant predictor of health status in later years. Being connected to other people and feeling valued is a goal everyone hopes to achieve, regardless of their age. In the survey, we asked 14 questions designed to give an overall sense of the social connectedness of residents and insight to better understand how well this connection has been achieved in a care home setting.

Staff Responsiveness

There is a diversity of physical needs in care homes. Some residents require little or no physical assistance to undertake any activity, while others require total care and need staff to assist them with all activities of daily living, including intimate personal care. The availability of staff to respond when needed, as well as the technical and emotional skill of staff, plays a large part in how residents experience day-to-day life. This set of questions explores residents and MFV perceptions on the responsiveness and ability of staff in meeting residents’ needs.

Caring Staff

In addition to feeling confidence in the skills and responsiveness of staff, it is important for residents to believe staff care about their health, well-being and happiness. Developing relationships with staff builds the foundation upon which residents and staff can engage and connect with each other, beyond the level of “caregiver” and “client.” Residents and MFVs were asked a series of questions designed to help give an overall picture of the level of engagement between residents and staff, as well as the involvement of MFVs in care decisions.

Food

Food is a very important part of daily life in a care home. One of the biggest adjustments some people make when they come to live in a care home is that choices around food are much more limited compared to when one lives in their own home, and that dining with a group of people for all meals creates a social environment that some will embrace more than others. It is within this complicated context that the following questions were asked to determine how well care homes have coped with meeting a diverse array of wants and needs.

Medications

The use, and potential misuse, of medications in residential care homes has been an ongoing area of focus for the Office of the Seniors Advocate. In its annual British Columbia Residential Care Facilities: Quick Facts Directory publication, the Office reports, for each care home in the province, the proportion of residents prescribed antidepressant medications, as well as the proportion taking antipsychotic medication without an accompanying diagnosis of psychosis. Empowering residents and their MFVs to ask about medications, along with the requirement of physicians to seek consent from either the resident or their designated decision maker, is acknowledged as the first line of defence in reducing both medication errors and over-medication.

Hand Washing

Residents in care homes are very susceptible to infections due to the communal living environment. The risk of complications from infections is also increased in the senior population, as immune system function declines with age. Hand hygiene is one of the most effective and easiest actions that can be taken to reduce infections. Visitors to care homes need to be educated and supported in the importance of hand washing, and directed on how and where to wash their hands when visiting the care home. Family Council

Family councils at care homes are composed of residents’ relatives or representatives. One of the key roles that a family council can play is to promote improved communication and collaboration between family members and a care home’s staff and management.
